I hereby waive and release Johnny Monell Jr. T/A 220 Second to None LLC. from any and all liability for any injury or illness incurred throughout the duration of baseball program, facility rental, individual/group lesson, camp. I have no knowledge of any physical impairment that would affect my child’s participation in any and all aspects of participation for baseball workouts, lesson or camps. I expressly agree to release and discharge Johnny Monell Jr. T/A 220 Second to None . And all affiliates, employees, representatives, successors or assigns, from any and all claims or causes of action. This waiver and release of liabilities, without limitation, covers all injuries which may occur. Regardless of negligence, as a result of (a) use of any and all equipment, products and amenities, (b)   the sudden unforeseen malfunctioning of any equipment, (c) instruction or supervision, and (d) slipping and/or falling while on the property, including adjacent sidewalks and parking areas. I acknowledge that I have carefully read this waiver and release and fully understand that this a release and fully understand that this is a release of liability. I agree to give up the right that I may to bring a legal action against  Johnny Monell Jr. T/A 220 Second To None for negligence, or any other personal injury or property damage of loss action.
